Project Description

As part of Age Friendly Island Age UK IW and Wight Home Care are working in partnership to provide Community Navigators across the Isle of Wight.

Community Navigators

Wight Home Care Community Navigators tackle social isolation for adults 50+ living on the Isle of Wight, the project works with people who would like to get out to community activities. Gain free help and support in the following ways:

  • Self referral/Professional referral
  • Signposting to community activities
  • Help organisations with activity ideas
  • Supporting activities at the venue

Clients will gain help with getting out to social activities by:

  • Initial get together to understand the person and their interests
  • Follow up visits building relations and arranging potential outings
  • Jointly attend social activities and promote confidence to achieve
  • Creating independence for client to continue organising themselves

Volunteers are a key part of the project, offering as little or as much time as they choose, please ask for further details if interested.
