Project Description

As part of Age Friendly Island Age UK IW are working in partnership to provide Care Navigators across the Isle of Wight.

Care Navigators are part funded by CCG (http://www.isleofwightccg.nhs.uk/) and My Life a Full Life (http://www.mylifeafulllife.com/)

Care Navigators

Age UK IW Care Navigators are based in GP’s surgeries and offer free and impartial advice to assist people aged 50+, as well as families and carers, with any challenges they may face. Visiting people in their own homes, they listen to what is important to the individual and discuss how to achieve their goals, and provide support to access appropriate services within Age UK Isle of Wight and partner agencies to best meet the person’s needs.Care Navigators can work with people for up to 6 visits. They promote independence, self-confidence and self-management to increase a person’s sense of health and wellbeing.

These visits support older people with accessing a wide range of services which can include:

  • Social activities
  • Day care
  • Domestic support
  • Falls prevention
  • Benefits and housing issues
  • Home safety
  • Minor aids and equipment for the home
